Heartland Women's Healthcare, an Advantia Health partner is GROWING and looking to add a member to our team. We are seeking a full-time Medical Records Clerk to join our growing healthcare team! This role is a remote position.

Summary/Objective

Maintains patient files and statistics. Responds to requests for medical records. Creates new medical records and retrieves existing medical records by gathering appropriate record folders and contents; assigning and recording new record numbers; verifying existing record numbers; inputting and recording locations to computer; delivering records.

Job Responsibilities:

  • Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
  • Gathers patient information by collecting demographic information from a variety of sources; interacting with registration areas and other physician offices; retrieving information from automated printer.
  • Maintains master patient index by completing assigned portion of daily audit trail; corrects and communicates problems according to established procedures.
  • Initiates the medical record by creating and processing the patient care record folder.
  • Maintains record availability by processing charts into the department; using chart mark-off procedures; facilitating chart location activities.
  • Processing medical record releases in a timely manner.
  • Retrieves medical records by following chart-our procedures; documenting reasons charts cannot be retrieved for statistical and follow-up purposes.
  • Keeps health care providers informed by communicating availability or unavailability of the record.
  • Maintains quality results by following practice standards.
  • Maintains continuity of work operations by documenting and communicating actions, irregularities, and continuing needs.
  • Maintains patient confidence by keeping patient records information confidential.
  • Serves and protects the practice by adhering to professional standards, practice policies and procedures, federal, state, local requirements, and practice standards.
  • Enhances medical records and practice reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.
  • Scan in old paper charts, all loose papers, and explanation of benefits into the admin file.
  • Submit all prenatal information to appropriate hospitals.
  • Responsible for the release of records.
  • Answer overflow phone calls and make appointments, answer patient questions, etc.
  • Interface with outside clinics, hospitals, insurance companies, law firms and patients regarding medical records.
  • Document and keep log records of all contraceptive devices.
  • Being accessible to the providers and staff for medical record requests.

Qualifications:

  • High School diploma or equivalent.
  • Customer service experience preferred.
  • Good written and verbal communication skills with a strong attention to detail.
  • Ability to work independently.
  • Excellent competency with computer navigation and data entry.
